Mainstream and SEND transport applications
Apply for travel assistance under mainstream and SEND criteria via the Citizen Portal.

If your child has a specialist healthcare need you may also be asked to complete the travel care plan. If you do not complete this form and a care plan is needed, it will delay your application.
Download and complete the travel care plan form (PDF) and submit the completed travel care plan with your application form on the portal.
The transport team will notify you of the outcome of your application within 10 working days of receipt of a correctly completed form, this outcome notification will be in your messages section when you are logged in to the portal.

Grove Academy school transport scheme
Please email transportplanning@slough.gov.uk to request an application pack. The pack includes the full eligibility criteria, terms and conditions, and an application form. There is currently no online application process.
Post 16 travel assistance
Post 16 pupils can apply for a student reduced fare card which enables 16–18-year-old students attending full time further education to travel on First buses at under 16 prices. The pass will not be valid on any other bus operators’ routes. Please complete the reduced fare card form and submit supporting documents by post to: Home to School Transport Team, Slough Borough Council, 25 Windsor Road, Slough SL1 2EL reduced fare card form (PDF).
Lost or damaged travel pass
