Your healthcare professional or social worker can arrange for your equipment to be collected.
You can contact Adult Social Care:
- by phone 01753 475 111, Option 1 for Adult social care or
- online via the Adult social care portal.
You will need to:
- give your name, address, date of birth and contact details and
- let us know what items you want to return.
