Paper applications are no longer accepted.
What you need before completing the online form
Before you apply for a licence you will need the following documents completed and in an electronic format to upload into the online form:
- part II application and medical declaration - completed by parent or legal guardian
- consent letter from headteacher on headed paper - if time off from school is required.
You will also need the following:
- a copy of the contract, draft contract or other documents (where they exist) containing details of the agreement regulating the child’s participation in the performance or activity to which this application relates.
- a copy of child's passport or birth certificate
- a passport style photograph of the child, taken within the last 6 months
- risk assessment or assessments – fully comprehensive and location specific including Covid-19 procedures
- production company's child protection and safeguarding policy.
Online form
Processing and timescales
Current legislation for child performance licence applications requires 21 days notice. However, we do attempt to process applications within 10 working days (not including weekends or bank holidays).
Validity of licence
A separate licence is needed for each production, set of performances or modelling assignment.
