Reablement and Independence Service
The Reablement and Independence Service is here to help eligible individuals, as determined by Slough Adult Social Care, regain and maintain their independence. We work closely with our service users and informal carers, setting rehabilitation goals with the help of the Occupational Therapy Service.
To reduce the need for care, our interventions involve using:
- assistive technology
- therapeutic interventions from an occupational therapist or a physiotherapist
- equipment
- home adaptations and
- community resources.
We're committed to collaborate with health partners and stakeholders to improve outcomes for Slough residents. Our goals are to:
- support people after hospital stays and also to:
- prevent unnecessary hospital admissions and care home placements and
- boost individuals' ability to manage their own needs.
We not only provide assistance, but also aim to empower our service users to lead independent and fulfilling lives.
