As a carer you have the right to ask for a carer’s assessment. This will look at the impact your caring responsibilities have on your life and what social care support you might be eligible for.
Slough Borough Council and Berkshire Healthcare Foundation Trust, Community Mental Health Team and the Older People's Mental Health Team will have an inclusive strength based conversation.
It aims to:
- support people to recognise that they are a carer
- start by discussing with carers what they can do with their skills and available resources
- support carers to develop and maintain their relationships with friends, family, other people and organisations in their communities to help them in their caring role.
A support plan may be developed and might result in a one off payment based on your assessed needs. If you have longer term needs we will look at the support available to you.
As with all adult social care services, sometimes services do have a cost, we will discuss this with you at the time of the assessment.
You can opt to have a joint assessment, as a carer with the person you care for.
