The Children Act 1989 requires every authority to establish and maintain a register of disabled children in their area. In Slough, it is the interagency disabled children and young people's database.
It has information on disabled children's personal circumstances and their service needs. Data on it is gathered by working closely with the following people:
This data is kept on a computer and is not linked into any other IT databases held in the borough. It is all protected by the Data Protection Act 1998 and is treated as confidential.
Being on the database does not directly affect the child's or the family's entitlement to services or benefits. It will only be used statistically to assist in monitoring current services used by families on the database. And to plan and give future services for disabled children in Slough.
Parents and carers are under no obligation to include their child's name on this database. We do encourage parents to do so as it helps with planning services for all children in Slough.
Young people whose names are on the database and their carers will, if they wish, receive regular information about disability-related services and events.
To register, call the database co-ordinator on 01753 690908 or print and complete the disabled child and young people's database form below.
